Checklists for opening and running a behavioral health program
Four printable checklists built from the roadmap, document lists and templates we use with clients and in our own licensed Texas PHP, IOP and outpatient program: every step to open a Texas IOP or PHP, every document insurance plans ask for, every form in an intake packet, and every item in a staff credential file. Tick them on screen (your ticks stay in your browser) or print a clean copy; only the checklist prints.

Which one first
- Planning a program? Start with the IOP opening checklist; the other three each cover one of its phases in detail. The story behind it is how to start an IOP or PHP in Texas.
- Already licensed and applying to plans? The credentialing documents checklist, then our credentialing service if you'd rather hand the folder over.
- Getting ready for a state or accreditation survey? The intake packet and staff file checklists cover two things surveyors always pull. Comply's chart audit checklist and Joint Commission checklist cover the charts and the standards.
- Admitting clients? Quick VOB's VOB checklist lists what to verify before each admission.
- Stuck on a word? The behavioral health business glossary defines IOP, PHP, ASAM, QCC, CAQH, NPI, SCA, UB-04, Part 2, EKRA and more, each with a link to the deeper guide.
How these checklists work
Each one is a page you can link to, tick on a phone or computer, and print. Ticks are saved only in your own browser; nothing is sent to us. "Print with my ticks" prints your progress; "Print a blank copy" prints an empty one for a planning meeting or a new hire's file. Without JavaScript the boxes still tick and the page still prints the checklist alone. Fees and rules change, so items that depend on them say to confirm the current version with the agency or plan.
