UnitedHealthcare behavioral health credentialing through Optum: therapists, groups and IOPs
UnitedHealthcare's behavioral health network is run by Optum Behavioral Health, not by UHC's medical network. To join, get a One Healthcare ID, make sure your CAQH profile is complete and attested, then start a network participation request on Optum's Provider Express "Join our network" page by choosing your provider type: individual clinician, group, or agency and facility (where IOP and PHP programs apply). Optum decides partly on network need in your area. Plan on 60 to 120 days once the file is complete, and recredentialing every three years.
Below is each step for Texas clinicians, groups and programs, sourced to Optum's and UnitedHealthcare's own materials, from a team that runs a licensed Texas PHP, IOP and outpatient program.
Updated October 24, 2026. Optum and UnitedHealthcare documents checked October 24, 2026. Optum is moving behavioral health prior authorization into Provider Express in late 2026; confirm current steps on providerexpress.com before you file.
Part of our credentialing guide for Texas practices and programs. Other plans: Cigna / Evernorth · Blue Cross and Blue Shield of Texas · Aetna.
UnitedHealthcare and Optum at a glance
- Who credentials behavioral health: Optum Behavioral Health, UnitedHealth Group's behavioral health business. Its provider site is Provider Express.
- Where to apply: The Join our network page on Provider Express, with a separate button for each provider type.
- Sign-in: A One Healthcare ID, the same login UnitedHealthcare uses on its medical provider portal.
- Credentialing data: From your CAQH profile (now DataSpring), which UnitedHealthcare's training materials ask you to have current and attested before you request to join.
- Malpractice: Optum's national network manual (2020 edition, updated 2021) set $1 million per occurrence and $1 million aggregate for master's and doctoral clinicians, and $1 million and $3 million for physicians, unless state law says otherwise.
- IOP / PHP programs: The agency and facility route on the same page, tracked by the Provider Reference Number Optum issues when you submit.
- Recredentialing: Plan on every three years, the cycle Texas requires of HMOs; re-attest CAQH every 120 days.
Which UnitedHealthcare you're joining
A UnitedHealthcare card in Texas can sit on several different products, and an Optum agreement doesn't automatically cover all of them:
- Employer and individual commercial plans. UnitedHealthcare's commercial prior authorization requirements (effective October 1, 2026) say many plans cover mental health and substance use services only through a designated behavioral health network, and tell providers to check the member's ID card. For most UHC commercial members that network is Optum's. Some self-funded employers carve behavioral health out to another company; our sister product's guide to behavioral health carve-outs shows how to spot it.
- Medicare Advantage. Enroll in Medicare through PECOS first (LPCs and LMFTs can since 2024; see our Medicare guide for counselors and MFTs), then ask Optum in writing whether your agreement includes UHC's Medicare Advantage plans.
- UnitedHealthcare Community Plan of Texas (Medicaid and CHIP). A separate network: enroll with Texas Medicaid through TMHP first, then credential with the plan. Our Texas Medicaid credentialing guide covers the order.
- Prescribers' medical visits. A psychiatrist or psychiatric NP billing E/M visits may also deal with UnitedHealthcare's medical network, which has its own credentialing process on uhcprovider.com. Ask which side your services fall under before you file twice.
Two portals and one sign-in
New UHC providers lose weeks to portal confusion. The short version:
| Portal | What it's for | Sign-in |
|---|---|---|
| Provider Express (providerexpress.com) | Optum behavioral health: joining the network, credentialing status, your practice information, behavioral claims and, increasingly, behavioral prior authorizations. | One Healthcare ID, from the "First-time User" link on Provider Express. |
| UnitedHealthcare Provider Portal (uhcprovider.com) | UHC's medical network: medical eligibility, claims and prior authorization, and medical credentialing. | The same One Healthcare ID. |
- Check your participation on Provider Express under My Practice Info, then My Networks, once you're registered.
- Behavioral prior authorization is moving. A UnitedHealthcare October 2026 notice says mental health and substance use prior authorization requests will soon be submitted through Provider Express; a request started in the UHC Provider Portal's prior authorization tool will send you to Provider Express to finish. UHC asks organizations to connect to Provider Express now with their existing One Healthcare ID. The change doesn't apply to Surest or Rocky Mountain Health Plans.
Set up the One Healthcare ID under the practice owner or administrator, not a departing biller's personal email. It's the key to credentialing status, claims and authorizations, and recovering it later is a project.
Individual clinicians, step by step
- Finish and attest CAQH. UnitedHealthcare's 2024 behavioral health training deck asks that all CAQH information be current and attested before you start. CAQH rebranded as DataSpring in June 2026 (Fierce Healthcare); the profile, login and 120-day attestation didn't change. Our CAQH ProView guide covers every section; authorize UnitedHealthcare/Optum to view it.
- Register for a One Healthcare ID through the First-time User link on Provider Express.
- Start the request. On the Join our network page, choose the individual clinician button and complete the network participation request. In Texas the independently licensed types are LPCs, LMFTs, LCSWs, psychologists, psychiatrists and psychiatric nurse practitioners; associates practicing under supervision generally can't hold their own contract.
- Make the network-need case. Optum, like every plan, can decline a clinician because it already has enough providers in an area. Say what you add: languages, populations (adolescents, veterans, substance use and co-occurring care), evening and telehealth hours, and the Texas counties you serve.
- Track the file. UnitedHealthcare's credentialing training materials say individual clinicians can follow initial credentialing on Provider Express with their One Healthcare ID; agencies and facilities use the Provider Reference Number issued with their request.
- Review the agreement before you sign: rates for your codes, timely filing, termination terms, the products covered and the effective date. Don't see UHC members as in network until that date is confirmed in writing.
Malpractice limits
Optum's national network manual (the January 2020 edition, updated March 2021, as posted by the Maryland Department of Health) set these minimums. They're the most recent figures we could verify from an Optum document; check the current manual on Provider Express before you buy or renew a policy.
| Provider | Per occurrence | Aggregate |
|---|---|---|
| Master's-level and doctoral-level clinicians (LPC, LMFT, LCSW, psychologist) | $1 million | $1 million |
| Physicians (psychiatrists) | $1 million | $3 million |
| Where state law requires different limits | State rule | State rule |
Upload the declarations page to CAQH, not just the policy number, and renew before the expiration date: an expired certificate stalls both initial credentialing and recredentialing.
Group practices
A group billing under its own Tax ID uses the group route on the same Join our network page. Each clinician still needs a complete, attested CAQH profile and their own credentialing, and each must be linked to the group's Tax ID and Type 2 NPI (see NPI Type 1 vs Type 2) before their claims will pay at the group's rates. Keep a roster with every clinician's NPI, license number and expiration, CAQH ID and start date, and send changes through Provider Express before the clinician sees a UHC member.
Opening a group? Our guide to starting a group practice in Texas covers the setup decisions, and the Group Practice Launch credentials every clinician and links them to the group's contracts.
IOP and PHP programs
A licensed program contracts with Optum as an agency or facility, not through the individual route. What we can say from Optum's and UHC's materials, and what to ask:
- Use the agency and facility button on the Join our network page and keep the Provider Reference Number Optum issues; it's how you check status.
- Accreditation and site visits. We couldn't confirm from Optum's public pages, on October 24, 2026, whether accreditation is mandatory for a Texas IOP or PHP or when Optum audits an unaccredited site. Ask your Optum network contact in writing before you plan around a contract. An accredited program has an easier path with Optum and most commercial plans (see Joint Commission vs CARF).
- Licensed for every level of care. Ask Optum for exactly the levels on your HHSC license, and no more; a level that isn't on the license is the first thing a reviewer catches. The licensing steps are in our guide to starting an IOP in Texas.
- Have the facility packet ready: the HHSC license for each level of care, the organization's Type 2 NPI, a W-9, general and professional liability certificates showing limits and expiration dates, accreditation letters and the latest survey report, the most recent HHSC inspection, the medical director's license, a staff roster with licenses, and your program description and daily schedule by level of care (our IOP and PHP schedule templates show what a payer expects to see).
- Then the contract: per diem rates by level of care and code (IOP H0015 or S9480 with revenue code 0905 or 0906; PHP H0035 or S0201 with 0912 or 0913), the claim form Optum wants, authorization rules, timely filing and the effective date. Our guide to insurance contracts for treatment centers covers what to check.
Not in network yet and a UHC member needs your program now? Ask Optum for a single case agreement for that client, with the rate, dates, codes and authorization in writing before admission.
How long it takes
| Stage | Timing | Whose number |
|---|---|---|
| One Healthcare ID and CAQH ready | A few days if CAQH is current | Our experience |
| Complete request to in network | Plan on 60–120 days | Our planning range, not an Optum promise |
| Texas HMO initial credentialing limit | 180 calendar days; decision sent within 60 days of being made | TDI (HMO plans) |
| CAQH re-attestation | Every 120 days | CAQH / DataSpring |
| Recredentialing | Every 3 years | TDI (HMO plans); confirm in Optum's credentialing plan |
Optum doesn't publish a single turnaround we could verify. The 180-day figure is the outer limit Texas sets for HMO credentialing committees (TDI's HMO credentialing requirements), not a target. Our guide to how long credentialing takes compares plans. Until your effective date, see UHC members as self-pay with a good faith estimate.
How we work an Optum file
This is the order we follow on our own program and for clients, and the checklist our client portal tracks.
| Step | What we do | Status you see |
|---|---|---|
| 1. Packet and sign-in | One Healthcare ID set up under the owner; W-9, IRS EIN letter, licenses, malpractice declarations page, Type 1 and Type 2 NPIs, a CV with month and year for every job. Programs add the HHSC license, accreditation, liability certificates and roster. | Gathering documents |
| 2. CAQH, then the request | CAQH complete, attested and authorized before the request goes in; the request says plainly what you add in your counties. We save the Provider Reference Number or confirmation. | Submitted |
| 3. Follow-up | Every two weeks, sooner when Optum asks for something, with each contact's date and reference logged. | In review · Payer asked for more |
| 4. Contract | Rates for your codes, products covered, timely filing, termination terms and effective date checked before you sign. You sign; we don't. | Approved · Contract to sign · Contract signed |
| 5. Go live | Effective date confirmed in writing and loaded into the EHR; My Networks on Provider Express shows the participation; group clinicians linked to the group's Tax ID and NPI. | In network |
You can see what this looks like in the client portal tour, with a made-up practice.
After you're in: authorizations
- Check the member's plan every time. UHC's prior authorization rules differ by product. Its 2026 behavioral health lists for exchange plans (for example Florida's) include intensive outpatient programs and substance use detoxification, while employer plans follow their own benefit design.
- Send behavioral requests through Provider Express as the October 2026 change takes effect, and connect your organization there now.
- Track each authorization's end date and send the continued-stay review before it lapses. Our prior authorization guide walks through it, and Cima EHR's program plan tracks authorizations and continued-stay reviews by client, with per diem billing for each level of care.
Recredentialing and upkeep
- Every three years. Texas requires HMOs to recredential participating providers every three years (TDI). Optum works from your CAQH profile, so a current, attested profile is most of the work; confirm the cycle in Optum's current credentialing plan.
- Re-attest CAQH every 120 days and keep UHC/Optum authorized to view it.
- Keep your practice information current under My Practice Info on Provider Express: locations, hours, telehealth, whether you're taking new clients, and specialties.
- Facilities keep the HHSC license, accreditation and liability certificates current with Optum, and report a new level of care or location before billing it.
Credentialing upkeep, $49 per clinician a month, covers CAQH re-attestation, license and malpractice renewals, recredentialing and roster changes for every plan, Optum included.
Why Optum applications stall
- CAQH not current, not attested or not authorized when the request goes in.
- Applying on the wrong side. A behavioral clinician filing with UHC's medical network, or a program filing as an individual instead of through the agency and facility route.
- No network need in your area for your specialty. Ask what would change the answer.
- Malpractice below the minimum or about to expire.
- A group clinician not linked to the group's Tax ID and Type 2 NPI, so claims deny after approval.
- A facility file that asks for a level of care not on the license, or arrives without accreditation documents and liability certificates.
- Services before the effective date or without an authorization the plan requires. Claim denials, not credentialing ones, but they cost the most.
If Optum says its network is full, get it in writing and read our guide to what to do when a network is closed.
What we do, and what it costs
Cima Consulting files Optum the way this page describes: One Healthcare ID set up under your practice, the CAQH profile built and attested, the request written to show what you add, follow-up every two weeks until there's a decision, a contract checklist before you sign, and the effective date confirmed in writing. You sign every application and contract yourself.
| Service | Price |
|---|---|
| CAQH ProView profile Your CAQH profile built from your documents, uploaded, authorized for your payers and attested. Includes: Profile built and documents uploaded; Payers authorized to view it; First attestation. | $250 per clinician |

A clinician with UnitedHealthcare/Optum is $175 per plan; a program's facility contract is $450 per plan. Opening a program from scratch? The IOP / PHP Program Launch includes facility contracts with your first plans.
Questions
Who credentials behavioral health providers for UnitedHealthcare?
Optum Behavioral Health. Clinicians, groups and facilities request to join on Optum's Provider Express site, not through UnitedHealthcare's medical network; Optum reads your CAQH profile and issues the agreement.
What is a One Healthcare ID?
The single sign-in for both Provider Express and the UnitedHealthcare Provider Portal. Register through the First-time User link on Provider Express, and set it up under the practice owner or administrator.
Does Optum use CAQH?
Yes. UnitedHealthcare asks that your CAQH profile (now DataSpring) be current and attested before you request to join, and Optum works from it when it credentials you. Re-attest every 120 days so it's ready at recredentialing too.
What malpractice coverage does Optum require?
Optum's national network manual (2020 edition, updated 2021) set $1 million per occurrence and $1 million aggregate for master's and doctoral clinicians, and $1 million and $3 million for physicians, unless state law requires otherwise. Check the current manual before you renew.
Does Optum require accreditation for an IOP or PHP?
We couldn't confirm a published rule for Texas from Optum's public pages. Ask your Optum network contact in writing before you count on a contract; accreditation by The Joint Commission or CARF makes Optum and most commercial contracts easier.
Where do I send UHC behavioral prior authorizations?
UnitedHealthcare announced in October 2026 that mental health and substance use prior authorization requests are moving to Provider Express. Connect your organization there with your One Healthcare ID, and check the member's plan for what needs authorization.
