How to start an IOP or PHP in Lubbock

Lubbock is the hub for the South Plains: a 15-county Medicaid service area that reaches north into the Amarillo area, one local authority that runs substance use access for 41 counties, a university town's client mix, and clients who may drive an hour or more each way. Here's what that means for opening a substance use intensive outpatient or partial hospitalization program, from operators who run a licensed Texas PHP, IOP and outpatient program.

Updated October 27, 2026. Not legal advice. Cima Consulting isn't a law or accounting firm; confirm building rules with the city and plan line-ups with HHSC before you rely on them.

The short answer: a Lubbock SUD program needs HHSC's chemical dependency treatment facility license, like any Texas program. The City of Lubbock's Planning Department confirms the zoning and Building Safety issues the certificate of occupancy; moving into a space with no valid certificate, or changing its occupancy classification, takes an investigative certificate of occupancy with a fire marshal inspection. The Lubbock service area has 15 counties; HHSC's August 2026 map lists Superior and Wellpoint for both STAR and STAR+PLUS. StarCare Specialty Health System is the local mental health authority and runs OSAR for HHSC's 41-county Region 1.

The license, from a long way off

HHSC licenses Texas substance use programs as chemical dependency treatment facilities under Health and Safety Code chapter 464; Lubbock adds no separate treatment license. As of October 2026 the fee under 26 TAC 564.408 is $1,200 plus $125 per additional site. Two habits help a program this far from Austin:

  • File a complete packet once. Back-and-forth by mail and email adds weeks. File only when the site, policies and staff files are nearly ready; the forms are in our CDTF license guide and the rule-by-rule proof list is 26 TAC 564 in plain English.
  • License each site. If you plan a second location in Plainview, Levelland or the Amarillo area, each address needs its own license ($125 each after the first) and its own certificate of occupancy from that city.

The full sequence is in how to start an IOP or PHP in Texas.

Zoning, Building Safety and the certificate of occupancy

Lubbock has zoning, and the city's own guidance puts it first: the Planning Department confirms the property is zoned for the proposed use and meets the Zoning Code, and every commercial business needs a certificate of occupancy, with the occupancy classification based on the type of business (City of Lubbock certificate of occupancy guidance). Then:

  • Building out? Commercial work needs a permit before it starts, and the certificate of occupancy is issued at the end of the construction or remodel permit.
  • Moving in without construction? If the space has no valid certificate, or your use changes its occupancy classification, you apply for an investigative certificate of occupancy. A commercial plan examiner tells you which documents to bring. After you pay, you set up the fire marshal's inspection, and the approved application and floor plan stay on site for inspectors to sign.
  • Issuance. Once the signatures are collected you return the application to Building Safety; the city says issuance can take up to five business days. The certificate has to be posted where the public can see it.

Get written answers to these before you sign a lease:

  1. Is an outpatient substance use treatment program a permitted use in this zoning district?
  2. Does the existing certificate of occupancy fit our use and occupant load, or do we need an investigative certificate?
  3. What will the fire marshal check, and which permits will the build-out need?

Lease with a contingency for the certificate of occupancy and the HHSC license. The site steps are on our Texas IOP opening checklist.

Medicaid plans in the Lubbock service area

Enroll with TMHP after the HHSC license, then credential with each plan; TMHP has said Medicaid SUD treatment may only be delivered in a licensed chemical dependency treatment facility (TMHP notice).

ProgramWho it coversPlans in the Lubbock service area
STARMost children, pregnant women and some familiesSuperior, Wellpoint
STAR+PLUSAdults with disabilities and adults 65 and olderSuperior, Wellpoint
  • Two applications cover both adult programs. Superior and Wellpoint are each listed in STAR and STAR+PLUS here, which makes Lubbock one of the simplest areas in Texas to credential for Medicaid. Superior's STAR+PLUS contract continued in Lubbock in 2024, and Wellpoint's STAR+PLUS footprint moved into Lubbock that year. See our Superior HealthPlan and Wellpoint Texas guides.
  • About FirstCare. FirstCare, the Lubbock-based plan owned by Baylor Scott & White Health Plan, appeared on earlier STAR and CHIP lists for this area. Baylor Scott & White Health Plan said it would leave Texas Medicaid managed care on August 31, 2026 (KWTX), and the August 2026 map lists only Superior and Wellpoint for STAR. If you see FirstCare on an older list, confirm with HHSC; our Baylor Scott & White Health Plan guide covers what's left of its business.
  • Adolescents. For STAR Kids and CHIP, read the area's lists on the map; children's programs weigh those more than STAR+PLUS.

The step-by-step is in our Texas Medicaid credentialing guide; for commercial plans, see insurance contracts for treatment centers. Our credentialing service files and follows the applications for you.

Serving a region, not just a city

Lubbock programs draw clients from small towns across the South Plains. That changes the program design more than anything in the permit process.

  • Schedule for the drive. A three-hour IOP block three times a week is a big ask for someone driving from Brownfield or Plainview. Evening tracks for working clients, and fewer, longer days for PHP, keep attendance up. Our IOP schedule templates show the options.
  • Telehealth, within the rule. 26 TAC 564.911 lets you counsel by electronic means if you verify identity at the start, record consent for each service, give written risks first, and use audio-only only with an existing clinical relationship and a documented reason. Check each plan's telehealth policy before you build a remote track.
  • Medicaid follows the client's county. A client from Cochran, Bailey or Yoakum County may live outside the Lubbock service area, in a rural service area with different plans. Verify every admission's plan first; our sister site's VOB checklist lists what to ask.
  • Students on family plans. Texas Tech students often carry a parent's employer plan issued in another city or state, which may have a different network and a behavioral health carve-out. Verify benefits and prior authorization before admission.
  • Weather. Severe thunderstorms, hail, blowing dust and winter storms can close roads from the outlying towns. Put them in your hazard vulnerability analysis and decide in advance when groups move online.

The local landscape

Earn referrals with fast assessments and honest updates. Paying or giving anything of value for referrals is illegal under the Texas Patient Solicitation Act and EKRA (patient brokering laws, explained). We never arrange referrals.

  • StarCare Specialty Health System is the local mental health authority for Lubbock, Lynn, Hockley, Cochran and Crosby counties, and HHSC's directory lists it as the OSAR provider for Region 1 (HHSC LMHA directory). OSAR screens people and links them to state-funded substance use treatment (HHSC OSAR page). StarCare also runs Sunrise Canyon Hospital, an adult inpatient psychiatric unit. Agree on crisis hand-offs, and know where to send state-funded clients if you don't take them.
  • Hospitals and the medical school. University Medical Center, Covenant Health and Texas Tech University Health Sciences Center's clinics see patients who need a step-down level of care. Discharge planners want your levels of care, accepted plans and how fast you assess.
  • Courts and probation. If the county runs a drug court or other specialty court under Government Code chapter 123, reports to it go only with the client's written consent under 42 CFR Part 2.
  • Rural clinics and counselors across the South Plains refer clients who need more structure than weekly therapy. A one-page sheet with your schedule, telehealth options and accepted plans does more than lunches. Legal marketing is in how to market a treatment center.

Questions

Which Medicaid plans serve Lubbock?

HHSC's August 2026 map lists Superior HealthPlan and Wellpoint for both STAR and STAR+PLUS in the Lubbock service area, which has 15 counties including Lubbock, Hockley, Potter and Randall.

Who is the OSAR provider in Lubbock?

StarCare Specialty Health System, Lubbock's local mental health authority, is listed as the OSAR provider for HHSC Region 1, which covers 41 Panhandle and South Plains counties.

What is an investigative certificate of occupancy in Lubbock?

It's the route when you occupy a commercial space that has no valid certificate, or when your use changes the occupancy classification, without a construction permit. You apply through Building Safety, schedule the fire marshal's inspection, collect the sign-offs and return the application.

Can a Lubbock IOP serve clients by telehealth?

Yes, within 26 TAC 564.911: verify identity, record consent for each service, give written risks first, and limit audio-only counseling to existing relationships with a documented reason. Payers set their own coverage rules, so check each one.

How long does it take to open an IOP in Lubbock?

Plan on nine to twelve months to a full caseload of insured clients. With two Medicaid plans covering both adult programs, Medicaid credentialing is shorter here than in most metros.
